The Meningococcal Vaccine for Travel — Who Needs It and Why

The meningococcal vaccine doesn't come up as often in travel conversations as Hepatitis A or Typhoid, but for certain destinations it's one of the more important ones. Here's who actually needs it.

What the meningitis belt is

There is a band of sub-Saharan Africa sometimes called the meningitis belt, stretching from Senegal and The Gambia in the west through to Ethiopia in the east. It includes countries like Mali, Burkina Faso, Niger, Nigeria, Chad, Sudan, and others. In this region, epidemic meningococcal disease (caused by Neisseria meningitidis, typically serogroup A) occurs in seasonal outbreaks, particularly during the dry season from December to June when dusty winds facilitate spread.

Meningococcal disease can kill within 24 hours of symptoms appearing. Vaccination is strongly recommended for travel to this region.

Saudi Arabia and Hajj/Umrah

Saudi Arabia requires meningococcal vaccination as a mandatory condition of entry for Hajj and Umrah pilgrims. The specific vaccine required is the quadrivalent ACWY vaccine and it must have been given within the last 3–5 years depending on type. This is an absolute hard requirement — no certificate, no visa for pilgrimage travel.

Which vaccine

The standard travel meningococcal vaccine is the quadrivalent ACWY vaccine, which covers four serogroups. There is also a Men B vaccine, which is part of the childhood immunisation schedule in many countries. For travel, the ACWY is the relevant one for most destinations. One dose is generally sufficient for healthy adults.

Who else should consider it

Beyond the meningitis belt and Saudi Arabia, meningococcal vaccine is worth considering for students going to university abroad (particularly the UK and US, where meningitis outbreaks in student populations occur), backpackers in close communal living conditions, and anyone with certain immune system conditions. Your travel clinic will flag it if relevant to your itinerary.
